For patients using United Concordia™, Cigna, Blue Shield™ of Pennsylvania, and Delta® Dental, we will be glad to help you obtain the appropriate benefit coverage from your insurance carrier and bill your carrier. However, you are responsible for any payment not covered by your insurance.
For patients who have dental insurance that the office does not participate with, the patient would pay the office directly for their dental services. The office will fill out the insurance forms and the patient is reimbursed directly from their insurance carrier.
Sometimes there is a deductible and/or a co-payment required from you as stated by your agreement with your insurance company. This deductible and/or co-payment is to be paid at the time the dental care is performed. We will be happy to request a pre-estimate of benefits from your insurance carrier if you request us to do so. Routine treatment is generally performed without submitting a request for pre-estimated benefits. We accept different Medicaid programs as well, so please contact our office to see if we can work with your plan.

Patients Without Insurance Coverage
Patients without insurance coverage are to pay for services as rendered. We accept checks, cash, MasterCard, Visa, Discover and debit cards as payment. Checks must be accompanied by a copy of a valid driver’s license or similar identification showing date of birth and address. Checks returned by your bank are subject to a $35.00 processing charge.
Patients Who Have Dentures and Partials Fabricated
These are created over a four-step process, and patients are required to pay on each visit. Dentures and/or partials are to be paid in full when completed. For one-day denture service, full payment is expected at the time of service.
A copy of the HIPAA Notice of Privacy Practice will be presented to you for your review. We are required by law to maintain the privacy of, and provide individuals with, the HIPAA notice of our legal duties and privacy practices with respect to protected health information.
